The patient had been experiencing chest pain and dyspnea on exertion since early XX, 20XX, and transported to Seirei Mikatahara General Hospital because of sudden onset of dyspnea at rest. When he came to the hospital, his heart rate was 100 bpm, blood pressure was 86/67 mmHg, and SpO2 was 94% with 10 L of oxygen. Coarse crackles were identified in all bilateral lung fields.
